The start date shown on your AF Form 469.

The end date shown on your AF Form 469, if it has one.

The form itself

Before you take the photo — cover your SSN.

Put a sticky note or marker over the Social Security Number block on the form. We never ask for your SSN, and we don't need it to process your profile.

Take a photo or choose a file. Photos are converted to a PDF automatically.

Up to 3 pages. On a phone you can choose Camera to photograph the form, or pick a file you already have. Photos are turned into a PDF for you.
